If this is a new belt that keeps coming off, then if not catching something in the roller like a scatter rug, etc then the belt isn't tracking true. The pulley may have come loose for one thing, something has to be off to have the belt come off. Does it come off almost right away, or after some uasge?
The belt started to slip off the main drive belt to my U6420-900 this morning, and after reinstalling it a couple of times, it came off again. I tried a new one, but after approximately 7 minutes of operation, it also slipped off. After I read your similar problem with no solution, I started to check the alignment of the wheels, the roller, etc. then I noticed that the belts after a certain amount of use, they stretch, and become loose, I compared it to a new one and found a difference in size.
\015\012My solution was very simple. I took the old belt and cut off a piece of approximately ¼ inch, and then I reattached the ends by using industrial glue (Ethyl Cyanoacrylate). After approximately half an hour I finished vacuuming my entire house the vacuum cleaner is still working.

The belt on my used Eureka Boss 5857 upright slips off the end of the motor shaft every time the vacuum is used. When I turn the brush roll slowly by hand, the belt walks toward the end of the shaft.
